Exit & Check-Out Procedure

For Graduating/Terminating Housestaff

The program/department sends prior notification to the GME office of those whom are expected to terminate at the end of the academic year and thus coordinates exiting procedures for these individuals. Before receiving a Training Physician Clearance Form*, Housestaff who are confirmed to terminate (also includes LA County employed trainees becoming LA BioMed and LA BioMed employed trainees becoming LA County) must complete the following:

1. “Confidential Institutional Housestaff Survey” evaluation in MedHub –  assigned by the GME Office (access available upon notice from GME toward the end of the year). This will found in the queue of tasks to do in the upper left-hand box once you log into MedHub under Incomplete Evaluations.

2. All terminating physicians must complete the Grade A (All Housestaff) and Grade B (Interns/Residents)/ Grade BF (Fellows) Institutional requirements. Please contact your program coordinator if your courses are locked or you have log-in issues.

GME Competency Education Program (GCEP)

Grade A online courses are required by the institution to be complete by all incoming residents prior to starting training at Harbor-UCLA.

Grade B: Residents will have 24 months of their residency training to complete the Grade B required courses. If the Residency/Internship is only 12 months the Grade B courses are required to be complete within the 12 months of training.

*Only until the survey is complete and all courses are taken with a passing grade of at least 80% each, may you proceed to the GME office (8E-21) to obtain the form. A single form will be released to each individual and in person, not in someone else’s stead.
