[dropcap color=”” boxed=”no” boxed_radius=”8px” class=”” id=””]O[/dropcap]n March 7, 2015, the Harbor-UCLA Family Medicine Residency received the Patient Centered Medical Home (PCMH) Practice of the Year Award for 2015 from the California Academy of Family Physicians (CAFP) in recognition of its innovative Transforming Primary Care Fellowship and collective efforts of residents, faculty, fellows, and staff to implement the patient centered medical home model in the HUCLA Family Health Center located on Lomita Boulevard in Harbor City. The clinic’s providers and staff are divided into three interdisciplinary quality improvement teams led by two fellows and a former fellow, now faculty member.

With the guidance of expert coaches and fellowship faculty, the teams work hard to make the care provided by the Family Health Center more effective and patient friendly. Funded by a five year federal grant from HRSA, the Transforming Primary Care Fellowship provides leadership training for recent residency graduates to learn leadership and quality improvement skills as well as principles of organizational transformation and more. The fellow-ship provides resources and training to support the PCMH transformation process and has enhanced the leadership and QI training of residents and staff. Harbor-UCLA Family Medicine is the first residency program to receive this award.
