Rotating/Visiting Resident– Check In Procedures

In order to rotate at Harbor-UCLA Medical Center the sending institution must have signed a PPG Affiliation Agreement with the County of Los Angeles. In addition a Letter of Agreement must be in place between the sending department and the receiving department.
All affiliating residents are required to register with the GME Office/Medical Administration at Harbor-UCLA Medical Center prior to beginning an affiliation (on the first day of rotation). This registration provides non-County employee malpractice coverage (if applicable) and hospital policy information helpful during the rotation. All affiliating residents also receive a copy of the current Housestaff Survival Manual which provides hospital phone numbers and other helpful information.
Prior to check-in, the program coordinator at Harbor-UCLA (click here for a list of Harbor's program coordinators) must submit to the GME office the Rotation List of Visiting Residents before you can get live scanned. Please contact the coordinator for the program you are rotating to verify that this list has been sent to Sally Oliver in the GME office. You will also be required to send the following documents to the GME office (Click Here for GME office location and contact information):
- Affiliating Resident/Fellow Questionnaire
- Code of Conduct - Download and complete our Test using the Study Guide. Correct your answers with the enclosed answer key. You must turn in the Answer Sheet and the Code of Conduct Acknowledgement to our office.
- HIPAA Certification: send/bring your HIPAA Certificate or complete our paper test. Download Harbor's Study Guide and Test. Please correct your answers with the answer key prior to submitting the Answer Sheet. UCLA Residents can print a certificate from the UCLA website: Click Here.
- Current California Medical License*
- Current DEA Certificate (if applicable)
- Valid ECFMG Certificate (for International Medical Graduates)
If you haven't done so already, you will also be required to submit an Employee Health Clearance form (E2) to the Employee Health office. Click Here to download the form and accompanying documents.. Please fax or email the E2 form to Erika Sweet in Employee Health at 310/222-5326 or esweet@dhs.lacounty.gov. You must do this at least a week prior to your rotation.
NOTE - if you already completed a Code of Conduct and HIPAA Cerfication, you are not required to complete this again.
IMPORTANT - you must also be live scanned before you can start. Please be sure to contact the Harbor program coordinator for the service you are rotating to to verify that the GME office has received the Rotation List of Visiting Residents. The GME office will then notify Human Resources that you will be coming for a live scan. There are three options for live scan:
Option 1 - Live Scan may be performed at the Harbor-UCLA Human Resources office located at the Medical Foundation Professional Building, 21840 S. Normandie (Suite 800) or at one of our other County locations. Click here for details on using Option 1 for Live Scan. Note - all arrangements for Live Scanning at another facility should be made through Esmeralda Guillen, at 310/222-7906 or at eguillen@dhs.lacounty.gov.
Option 2 - Live Scan may be performed at a "public" Live Scan agency. Click here for details on using Option 2 for Live Scan.
Option 3 - Ink Fingerprinting - Hard Card
Fingerprinting may be performed by a local law enforcement agency. With this option, the applicant is responsible for paying the rolling fees charged by the agency. In order to initiate this process, the applicant must contact the DHS Background Verification Unit (BVU) at 323/869-8242. The DHS BVU will mail a fingerprint card, the pre-employment packet, and the applicable instructions for completing the process. Since this process is conducted via standard mail service, we recommend that applicants choosing this option begin at least 2-4 weeks prior to their scheduled start date.
Note: If you have already been live-scanned and cleared Employee Health at another LA County site, you may not have to be cleared again at Harbor. Please contact Human Resources at 310/222-3241 to verify your live scan status prior to obtaining your live scan.
IMPORTANT - After you have obtained your live scan clearance (please check with the program coordinator or HR for status) and have provided the E2 form to employee health, you must check in with the GME office on the first day of your rotation to sign in. Once your credentials have been verified, you will be instructed to go to HR to receive a validated picture ID badge that enables you to receive meals and parking at no charge for the duration of your rotation. You will also be assigned a Physician ID# that you must write wherever you write your name (on orders, etc.). Please note: there is a $25 fee for a lost ID badge. White coats and/or hospital scrubs are not provided to affiliating residents.
* California state law requires that all U.S. Medical School graduates obtain licensure by end of the 24th month of training; foreign medical graduates have until the end of the 36th month of training. Trainees wishing to rotate at Harbor from outside the state, and who do not have a CA license, may be eligible to do so if they meet the requirements of the Institutional Policy for Resident Guest Rotations and complete the necessary paperwork.
Returning Rotating/Visiting Resident – Check In Procedures
All affiliating residents returning to Harbor-UCLA Medical Center must register in the GME Office/Medical Administration for each new rotation and complete a new Affiliating Resident Questionnaire. You will also be required to present renewal copies of any expired credentials listed above.
After your credentials have been verified, you will receive an updated picture ID badge from Human Resources that enables you to receive meals and parking at no charge for the duration of your rotation. You will be assigned the same Physician ID# you received on previous rotation(s). Please note, there is a $25 fee for a lost ID badge. White coats and/or hospital scrubs are not provided to affiliating residents.
