Primary Care Clinic

The Harbor-UCLA Medical Center Primary Care Clinics provide comprehensive, team-based care. We provide over 10,000 outpatient visits per year to a primary care population of about 12,000 patients. Residents are members of Attending -led, interdisciplinary Primary Care Teams including Nursing and Case Managers. The Teams assist with ongoing management of each patient’s care.

We serve a diverse primary care population that includes a large Managed Care population, the medically underserved in our community, and patients who seek tertiary care for rare and complex conditions at our Medical Center. As a result, our Primary Clinics offer exposure to an outstanding, balanced range of pathology and primary care management, from common chronic diseases such as diabetes and hypertension, rare diseases, and population-based preventive care measures for our Managed Care patients.

We provide comprehensive care to our patients. We have our own Women’s Screening Clinic, Procedure Clinic, and same day access to all of our empanelled patients. Additional services include in-clinic Case Managers, in-clinic Financial Services staff, and electronic referrals through eConsult that allows tracking outpatient referrals.

We are dedicated to quality improvement and resident input. Residents have conducted QI projects in our Clinics, such as on obesity management and foreign language access. For 2015-2016, Harbor Primary Care has partnered with Toyota’s LEAN Quality Improvement team in our transformation into a Patient-Centered Medical Home.
